Download & Submit Order Kit

Download and complete the ORDER KIT below and send it back to us. In it you will find the Order form, size charts and design templates, streamlining your order process!

Payment & Shipping

We will send you a sample prior to confirmation of order, after which we will require 50% payment to confirm your order and full payment before shipping of the order (est 3-5 weeks)

What you need to know

  • Please make sure the shipping details is complete with your zip code.
  • Orders require 4-5 weeks lead time. This includes processing the order forms, design, production, and preparing for shipping.
  • For design/ artwork assistance, please contact us.
  • Please ensure the correct UPPER & lower cases for jersey names.
  • Invoice will be sent after we have processed your order.
